It is bacteriostatic and is primarily active against Gram-negative bacteria. Its mode of action is inhibition of protein synthesis at the ribosomal 30S sub-unit level. Spectinomycin is not well absorbed by the oral route and the vast majority (>90%) is retained in the gut and excreted in the faeces.

Lincomycin and spectinomycin have been shown to be synergistic in vitro against anaerobes implicated in the pathogenesis of swine dysentery.
